At Sharp Community Medical Group, our caring staff of case managers is here to help guide you on your wellness journey. With telehealth video visits, it's easier to get the care you need when and where you want it, at no cost to you.

You can have face-to-face appointments with your Sharp Community case manager — who is also a registered nurse or social worker — from the convenience of your home using your phone, computer or tablet. Phone visits are also available.

About the visit with your case manager

Your case manager will work with you on meeting your desired health goals, coach on managing a chronic condition, coordinate care with providers and refer you to community resources based on your individual needs.

The visit with your Sharp Community case manager is private and protected by HIPAA.
